Coolsculpting Elite Candidacy

About Face Skin Care is looking for a model for a LIVE CoolSculpting Elite treatment at our upcoming event on March 13th at 5:30. If you are interested in being a model, please click on the form link below and answer a brief survey. We have outlined preliminary qualifications and stipulations before answering the questionnaire.

•⁠ ⁠THIS TREATMENT WILL BE FOR THE ABDOMEN ONLY.
•⁠ ⁠THIS IS A LIVE TREATMENT IN FRONT OF AN AUDIENCE.
•⁠ ⁠You must consent to AFSC photo/video policy that allows AFSC to use the photos for marketing purposes.
•⁠ ⁠Questionnaires are required and must be received by Monday, March 11 at 12 PM EST.
•⁠ ⁠You will need to be available for a 30-minute consult at our office in Snellville on March 12th (Exact time TBD)
•⁠ ⁠You will need to be available on March 13th at 4:45 PM until 8:30 PM for the actual LIVE treatment.
•⁠ ⁠Final patient selections will conclude by 5 PM EST on March 12th.
•⁠ ⁠Your LIVE treatment will be FREE. Any future treatment to get the desired results will be FREE and will be conducted inside our practice. Treatment is limited 8 CYCLES.
